An immunisation record and other evidence of vaccination are translated for their specific use. Certification, an apostille or legalisation is added only when it applies to the document and is required by the recipient.

An immunisation record documents administered vaccines and related medical entries. A translation may be required for education, employment, residence, healthcare or an administrative procedure in another country. The service is defined by the exact type of record, its issuer and the written requirements of the institution to which it will be submitted.
## Which documents can provide evidence of immunisation status?
Vaccination data may appear in an immunisation record, an outpatient medical record, a vaccination certificate, a certificate of immunisation status or an International Certificate of Vaccination or Prophylaxis. These titles are not interchangeable. The translation preserves the document's actual title and conveys its function without presenting one type of record as another.
## What must be reproduced accurately in the translation?
Particular care is given to the holder's names, the dates and sequence of doses, vaccine and disease names, manufacturers and batch numbers where recorded, as well as signatures, stamps and details of the health professional or medical facility. Abbreviations are rendered consistently and unclear handwriting is not completed by assumption. The translation reflects the record; it does not interpret medical status or replace an assessment by a health authority.
## When is a translation sufficient and when is a certified translation requested?
A professional translation of a legible copy may be sufficient for an employer, educational institution, doctor or insurer. In a formal administrative procedure, the recipient may request a translator's signature, a certified translation, a paper copy or a particular form of the source copy. These requirements differ from authentication of the source record itself and are checked separately.
## Why does the procedure depend on the document's issuer?
No. An apostille concerns the origin of a public document and is not automatically available for every medical entry. A vaccination card or a note from a medical practice may not be a public instrument, while a certificate issued by a competent health authority has a different status. Where formal authentication is required, the issuer, signature, country of issue and document accepted for the specific procedure are established first.
## How is this different from an international vaccination certificate?
The International Certificate of Vaccination or Prophylaxis under the International Health Regulations follows a prescribed model. It is completed in English or French and may also contain another language; its validity depends on correct completion, the signature of an authorised health professional and the official stamp of the administering centre. Such a certificate may reduce the need for a separate translation, but the applicable requirement is set by the receiving country and authority.
## How is a Bulgarian immunisation document prepared for use abroad?
The original title, issuer, legibility, all pages and attachments, stamps and signatures are reviewed together with the foreign recipient's requirement. If an ordinary immunisation record is insufficient for the intended purpose, a competent Regional Health Inspectorate may issue a separate certificate of immunisation status on the basis of the available records. Translation and any authentication are planned for the document that will actually be accepted.
## How is a foreign vaccination document submitted in Bulgaria?
For a foreign record, the country, issuing authority or health professional, language, means of verification and purpose of submission are relevant. A Bulgarian translation may be sufficient, while a formal procedure may call for additional authentication. The receiving institution decides whether the record proves the required circumstances; translation does not alter its evidential value.
